Welcome to the Sullivan Way Surgery
Through this new website we hope to improve our services to all our patients and enable you to find the information required at the click of a mouse. You can order repeat prescriptions online, download registration forms, cancel existing appointments, change your name and address and much more.
You can find out information about medical conditions and diseases through our patient information leaflets as well as keep up to date with all the latest practice news and developments. You can also allow the nurses to collect travel information by using the Travel Form.
We would be grateful if you could take the "Friends and Family test" which takes about a minute to complete. It is on the right hand side of the page. Thank you for your continued support.
From April 2015 all patients have been allocated a named GP responsible for your overall care, please contact the surgery if you wish to know who this is. If you have a preference as to which GP that is, then we will make reasonable efforts to accommodate this. This does not effect who you prefer to see in the practice.
We are currently identfying service users with communication needs as part of our service provision. This includes patients, parents, carers and any service users. Do you have a problem with communication:-
Do you need an interpreter? Do you require information in large print? Do you have sensory loss? Do you have any other communication needs? If so please let the receptionist know so we can make a note in your medical record and ensure you have the communication assistance you need.
Have a look around our website and please do send us some feedback. We can use your thoughts to improve our online services & further develop the website to make it a more useful, practical application for our patients.PLEASE NOTE: We cannot respond to comments through this website.
All consulting and treatment rooms are on ground level and corridors and doors are wide enough for wheelchairs. Please ask the receptionist for extra assistance if required.
Patients who have hearing problems can ask to speak away from the reception area.
If you have sight problems please do not hesitate to ask for assistance if necessary. It would be helpful if you could let the doctor or receptionist know if you are a carer.

CQC Report 2015
Patient Access has been in use in the surgery for some years now and many of our patients are happily using it to let us know that they have changed address, book appointments and order repeat prescriptions over the internet. It is our intention to extend our online services by offering patients access to their GP records by the 31st March 2016. We will be offering the facility for patients to view online, export or print information relating to medications, allergies, adverse reactions and other items as agreed between you and your GP. We will provide more information and notify you when this service is available. See information in the waiting room.
Q. What about children?
We will reduce the set age limit for Patient Access to 14 years and parents can manage the system for their children below that age.
